To receive the maximum benefit from your prescription Drug Benefit Program, please refer to the following guidelines.
Present your Prescription Drug Card each time you have a prescription filled. If you or your physician call in a refill order, remind the Pharmacist of your participation or present your card when you pick up the prescription.
The pharmacist will utilize the information shown on your card and electronically transmit the claim at the time of service. If you find it necessary, due to an emergency, to utilize an out-of-network pharmacy, you will pay 100% at the pharmacy and may file for reimbursement using a prescription claim form.
Reimbursement will be based on the allowed charges. Make sure your portion of the claim form and the portion to be completed by the pharmacist are accurate, then submit the claim to MaxCare for processing.
